Dermal bio-stimulation
Intradermal injection of drugs that promote the synthesis of collagen and elastin
Dietology
At our clinic there is a diet service, entrusted to nutrition doctors, for the treatment of overweight and obesity that also uses the molecular diet method. The molecular diet proposes a food style that allows you to lose weight, where there is an indication, or simply for a healthy life. The concept of calorie is superseded by that of the molecule: what matters to feel good or lose weight correctly, it is no longer counting the calories ingested daily, rather the types of food consumed and how they are combined with each other. In order to illustrate it the molecular method is based:
– on the choice of foods to control the glycemic, insulinemic and postprandial lipemic peak;
– on the intake of adequate quantities of molecules able to “modulate” our DNA, such as polyphenols and carotenoids rich in vegetables and fruit;
– on a weight loss that must occur by losing fat mass and preserving muscle mass, and must be gradual and continuous to achieve and then maintain the weight
Bio-revitalisation
Bio-stimulation consists in the administration of intradermal injection, with very thin needles, of substances that moisturise the dermis, give tone and elasticity, prevent skin ageing. The active ingredients used are usually hyaluronic acid, amino acids and vitamins. These substances can be compared to food supplements for cells: the water content that fills the intercellular spaces increases and the production of collagen and elastin is stimulated. In the field of aesthetic medicine, the main indications for skin bio-stimulation are represented by the phenomena of skin ageing and dehydration. The duration of the beneficial effects varies depending on the severity of the starting situation and in relation to the lifestyle that is led (for example, abstention from smoking and exposure to the sun and the constant use of sun filters favour a greater effectiveness over the period of treatment). In any case, like all cyclical treatments, it needs to be repeated over time and sustained by maintenance sessions. The areas most often treated are the face, neck, décolletage and the back of the hands, but the entire skin surface can be treated. The skin will appear more compact, hydrated and tense.
Peeling
Peeling (from the English to peel) means rapid and controlled removal of the outer layers of the skin, through the local application of chemicals, stimulating cell regeneration. The superficial and medium-superficial peeling (soft-peeling), through the removal of the corneous layer, immediately make the skin surface brighter, which also acquires greater smoothness. The indications are: skin ageing, skin spots, active or scarring acne, hyperseborrea, acne rosacea. The peeling is carried out in single treatments at intervals of about 15-30 days or other frequency based on the agent used. The number of sessions varies according to the target to be achieved, the chemical agent used and the individual response. With a soft peel it is reasonable to expect: greater shine, smoothness and elasticity of the epidermal surface; a slight reduction in the diameter of follicular hosts (the so-called “pores”); greater uniformity of skin colour, with attenuation of hyper-pigmentation (especially if the peels are repeated and associated with the use of depigmenting products), an attenuation of the finer wrinkles. It is not possible to predict the extent of these improvements, as the quality of the response and the number of treatments needed to obtain it depend on multiple factors, such as the type of initial impairment, age, individual responsiveness, environmental conditions and lifestyle.
Non-albative radio frequency
This is a method that represents one of the most modern techniques used in the treatment of anti-ageing and sagging of the face and body, giving a rested and “rejuvenated” appearance without the use of needles or drugs to be injected, therefore not very invasive. The electromagnetic energy generated by appropriate electro-medical devices, penetrates into the dermis and converts to heat, leading to an increase in the synthesis of collagen I and a rearrangement of the collagen fibres which shortens the consistency of the dermis: the tensor effect thus obtained gives a more toned appearance to the skin. The treatment is performed by sliding a handpiece on the skin surface, and can be performed on different areas of the face and body: eye contour, naso-genals furrows, cheekbones, forehead, neck, décolletage, arms, inner and outer thigh, buttocks, abdomen. The therapeutic protocol provides a cycle of 8-12 sessions with a frequency of once a week. Normally, 15-20 minutes are sufficient to perform a radio frequency session, with a reasonable variability in relation to the number and extent of the sites to be treated. The treatment is not painful: there is only a pleasant sensation of warmth.
Botulinum toxin
Botulinum toxin represented a “shocking revolution” in the history of Medicine and Aesthetic Surgery. Promoted, in fact, at the end of the 90’s as a drug for the treatment of wrinkles between the eyebrows, thanks to the natural nature of the result and the very wide margin of safety that this method allows, it has quickly conquered both doctors and patients. The use in aesthetics is authorised both by the strict American FDA and the European institutions. The method used today is accepted by the scientific community and allows results to be obtained with an optimal compromise between duration and naturalness. From a technical point of view, botulinum toxin is a drug that if injected into a muscle has the ability to reduce its contraction. This action has a fairly constant duration of about 3-4 months, after which the muscle returns completely to the initial condition. The experience accumulated over time has allowed us to develop the concept of treatment with botulinum toxin and to understand that in fact the most striking outcome of therapy is not so much the reduction or disappearance of wrinkles of the upper part of the face, as the constant improvement of the expression of the person, appearing calm and rested. Treatment with botulinum toxin can run from 2 to 3 times a year depending on the needs and is accomplished by tiny injections that are not normally very painful, distributed in the region around the eyes, forehead and between the eyebrows. Thanks to the natural result, the wide margin of safety (the largest among all the aesthetic medicine treatments) and the absolute safety of the drug, the treatment has become the most widely used in recent years.
Filler
Many facial dis-harmonies can benefit from the treatment with fillers used for the restoration of reduced volumes or the filling of furrows and wrinkles such as nasogenal furrows or wrinkles of the lip contour. For this purpose, hyaluronic acid is used in our Outpatient Service, which has the characteristic of being very well absorbed into the tissues in which it is injected and of filling them in a very natural way. The Italian Society of Aesthetic Medicine has been among the most active promoters in the Ministry of Health of guidelines on the safe use of fillers: at our clinic, therefore, we do not use permanent substances, but only fillers that can be absorbed. Hyaluronic acid is a disaccharide present in all human connective tissues, including the skin and the one on the market today and used in aesthetic medicine is the result of complex laboratory synthesis obtained with the help of biotechnology, so with minimal possibility of allergic reaction and characterised by excellent bio-compatibility. Once injected, the product is progressively metabolised and then reabsorbed by the body at variable rates depending on the area treated, the degree of skin ageing and the type of hyaluronic acid used.
Cavitation
Cavitation is a method of Aesthetic Medicine that uses ultrasound to treat accumulations of adipose tissue: the clinical indications are therefore localised adiposity (as sole methodology or in association with Liposuction) and lipomas (if there is a contraindication of surgery or if it is necessary to reduce the volume of the lipoma to be removed).The shock energy of the ultrasonic wave causes the fragmentation of the fat globule of the adipocyte, with the release of triglycerides. The treatment involves, after injecting the physiological solution into the areas to be treated, the external application of the ultrasound with a probe and then a slight massage of the infiltrated area. The treatment frequency is usually weekly for a number of sessions ranging from 10 to 12.
Manual lymphatic drainage
This is a series of manoeuvres performed manually, with a slight pressure and at a slow pace, which allows the localised swelling on the upper and lower limbs or on the face to be reduced. It acts on the accumulation of water in the tissues, counteracting the edema due to a deficit of the venous system and of the lymphatic system.
The main indications of manual lymphatic drainage are lymphoedema, constitutional hypotonic phlebopathy, edematofibrosclerotic panniculopathy (the so-called “cellulite”) and post-traumatic and post-surgical edemas.
Endermology
Mechanical message to improve skin firmness and counteract the imperfections of the so-called cellulite
Carboxytherapy
Carbon dioxide in the gaseous state has the particular characteristic of promoting vasodilatation and restoring proper blood flow, thus stimulating cell regeneration. After treatment, the skin improves its appearance in terms of firmness and elasticity. It is recommended for cellulitis, district adiposity and skin rejuvenation.
University clinical studies and research have shown that carbon dioxide administered subcutaneously or intradermally has:
– an action on the micro-circulation, indicated in the treatment of cellulite, because it acts on the vascular component that characterises the micro-circulatory physiopathology of edematofibrosclerotic panniculopathy
– a lipolytic action, indicated in the treatment of district adiposity, because it acts on the adipose component in excess determining the lipolysis of intradipocytic triglycerides and therefore the reduction of adipose tissue
– an action on the skin, where in addition to increasing the percentage of oxygen in the tissues improving skin elasticity, induces a rejuvenation of the dermis.
The frequency of sessions is usually one per week for 8-10 sessions per cycle. The cycle can be repeated twice a year. The treatment duration is on average 15-20 minutes, and the return to normal activities is immediate.
This consists in administering drugs, by injection in an intradermal site, on cutaneous areas projecting from the site of the pathologies. On the treated area, more injections are conducted, each with a small volume of drug which, from the point of inoculation, is distributed by a slow diffusion, to nearby structures. In mesotherapy the same medicines are used in the official pharmacopoeia; the choice of the mesotherapy route is justified with the advantage of using minimal doses of drugs, whose local concentrations are maintained for a long time, creating an intense and prolonged local activity. The main indications for the mesotherapy treatment are represented by:
– the various imperfections linked to the chronic insufficiency of the peripheral veno-lymphatic micro-circulation;
– the edemato-fibro-sclerotic panniculopathy or PEFS (dystrophic alteration of the subcutaneous adipose tissue, commonly known as “cellulite”)
– the algic symptoms of the osteo-articular apparatus.
The extent of the response and the duration of the therapy necessary to obtain it depend, as always in medicine, on many factors, such as the type of pathology, its severity and development, the possible presence of other concomitant unhealthy conditions, age, individual responsiveness, lifestyle. Since most of the diseases treated with mesotherapy have a chronic course, including “cellulite”, it is important to repeat the therapeutic cycle at regular intervals and to implement, in between them, a maintenance therapy.
Bio-stimulation with Platelet-Rich Plasma (PRP)
The use of platelet concentrate (PRP – platelet rich plasma) for non-transfusion use, in particular in regenerative medicine and in aesthetic medicine is a practice now contemplated at an international level.
PRP (platelet-rich plasma) is a potent concentrate of growth factors that stimulate tissue regeneration. It is obtained thanks to a technique that involves centrifuging autologous blood (that is, taken from the same patient) within a process that ends with the production of a part of plasma with a high concentration of platelets.
For what purpose: for non-transfusion use, the platelet concentrate thus obtained, has been used successfully for many years, as a re-generator of damaged tissues, for example in maxillo-facial and odontostomatological surgery, in ophthalmology, in the treatment of chronic skin ulcers and in orthopaedics.
In Aesthetic Medicine: the blood taken from the person who wants to have the treatment is subjected to a process that allows the separation of the platelets from the other cellular components and to obtain a product with a high platelet concentration. This platelet gel is re-injected in areas where it is necessary to improve the skin’s trophism by means of a very thin needle: also treatable are the face, neck, décolletage and hands, as well as the arms, abdomen and inner thighs. The use of PRP stimulates cellular processes represented by the regeneration of the dermis, the formation of collagen, the neoangiogenesis, with an aesthetic improvement of the skin that appears more toned and elastic. The skin rejuvenation thus obtained will certainly further improve the effect and duration of any other treatment that will be performed on the patient. There are also excellent results in the early treatment of hair loss, inducing a neovascularisation in the scalp and the consequent stimulation of the regrowth of the hair bulb.
